Referral Program
How to get raffle tickets:
- If you are new patient with us: you will get 5 raffle tickets after your first complete exam and cleaning appointment.
- Refer a friend/neighbor/co-work: you will get $20 gift card and 5 raffle tickets.
- Refer your family: you will get 5 raffle tickets per each family member you referred.
We will draw a lucky winner every quarter and he/she will win Apple AirPods!

Qualification referral:
The person you refer MUST be a new patient in our practice who received treatments ("treatment" includes exam and cleaning).
Rules and Restrictions:
- A maximum of 1 gift card per family referred (i.e., if you refer a family of 6, you will be given a maximum of one $20 gift card and 5 raffle tickets for that referral.)
- No maximum raffle tickets when you refer your family.
- Gift card will be mailed or picked up after referred patients' first appointment.
